Client is responsible for completing the following insurance verification and WILL NOT be reminded about this later:
1. Click the insurance logo above or submit here for insurance authorization (this does not automatically approve coverage or schedule you. Once approved, you’ll need to return to BreastfeedingHousecalls.com to schedule your consult).
2. Provide insurance info is the TLN link to check if your specific plan (not all do) covers lactation consultations (while this takes minutes to do, it can take up to a day or two to hear back about approval; please plan your visit accordingly). You can choose your level of time-sensitivity on their link (emergent, non-emergent, etc.)
3. Return to BreastfeedingHousecalls.comto schedule consult (please do not schedule your consultation prior to checking for coverage unless you’re ok with private-paying).
What to bring: If you want us to observe a feed, bring a baby who is willing to feed (not super hungry and not super full). If you pump and/or bottle feed bring your pump and a full bottle. Bring your partners or support people, when possible.
Illness: If you or anyone in your household is sick, we greatly appreciate you rescheduling until everyone at home is well.
Siblings: We welcome children of all ages with proper supervision. We recommend bringing caregivers so mom can focus on our time together.
Strollers: Due to space limitations, we ask that you leave your stroller in your vehicles.
Accessibility: Our office entrance has about 5 steps. If you need wheelchair accessibility, please let us know prior to your arrival so that we may set up our ramp and guide you to the correct entryway.
